Questionnaire Pretest — Clarity, Order & Effort

Use after pretesting a questionnaire that is still being prepared. Identify the version, context, who will read the feedback, and how it will be used. Answers describe situated impressions; they do not prove fatigue or a causal order effect.

  1. Which version or part of the questionnaire did you review, and in what context?
  2. In your own words, what did you understand from the question or instruction that required the most attention?
  3. Which words, examples, or instructions seemed unclear to you? How did you understand them? If nothing seemed unclear, say so.
  4. Did an earlier question change how you understood a later one? If so, describe the sequence; if you are unsure, say so.
  5. Which part required the most effort or recall? If none stood out as demanding, say so.
  6. Was there a question you could not answer from your experience or knowledge? Which one, and why?
  7. Did any question feel repetitive, unnecessary, or difficult to answer comfortably? You may choose not to answer this question.
  8. What specific change should be tested first, and what part of your pretest motivates that suggestion?
